Completing the Textbook List

 

  1. Teachers should list the name of the student, condition of the book, and the book number in the appropriate places on each list.  One list is provided for each block taught.  If the subject has two books, teachers should use the column on the right for the second book.

 

  1. The following letter designators should indicate the age of the book.

 

    1. New book issued this year............................... A
    2. Book used for one year before this year........... B
    3. Book used for two years before this year......... C
    4. Book used for three years before this year........ D
    5. Book used for four or more years before this year…….E

 

  1. The following letter designators will apply for the condition of the book.  Place this letter beside each letter designating the age of the book. (See example)

 

a.       New book....................................................... E

b.      Good condition................................................ G

c.       Fair condition.................................................. F

d.      Poor condition................................................. P

 

  1. Teachers should be very careful in taking up books from students. Each student should sign the list that they have returned a textbook.  Each faculty member should account for every textbook in one of two ways; either present the book or a deficiency notice for a missing book.  Noting a missing book on the book form will not be enough.  Nothing else will be accepted.
